Transitions are a common and often challenging aspect for children who are unable to be cared for by their family of origin. Moving a child into a new home is a life altering event. Each transition plan is not only unique to each child-youth, and how they will experience it is also unique. When a plan is made for a child to transition to a new family through adoption, it will be made in the best interests of the child and in collaboration with all who are involved in the plan.
